Analgesics and pain relief in pregnancy and breastfeeding

Women should be reassured that pain can be treated during pregnancy and lactation and that they need not suffer unnecessarily. Key words: codeine, non-steroidal anti-inflammatory drugs, opioids, paracetamol. If used appropriay, common analgesics such as paracetamol, aspirin, non.

Women who have used NSAIDs inadvertently during the first trimester should be reassured about the use, but other analgesics such as paracetamol should be recommended as preferable options for subsequent use. A suggested mechanism to explain the increased risk of miscarriage is interference with implantation as a result of effects on the prostaglandin pathway.

Paracetamol is the analgesic and antipyretic drug most widely used in Australia, particularly by pregnant women. Although it readily crosses the placenta in its unconjugated form, in therapeutic doses it does not appear to increase the risk of birth defects or other adverse pregnancy outcomes.

Also, I found I had to be on top of asking for the regular panadol as with the second c section the nurses pretty much left me to my own devices!!!. The trick is to have very regular pain relief even if it's just panadol. By day 5 post I stopped all meds as panadeine causes constipation and although they say the meds are safe with breast feeding I would just prefer not to have any! They can also be taken on a regular basis, but in that case an INR should be checked about 4-5 days after starting the drug, and again about 10 days later, to make sure that there has not been an interaction of the newly taken regular pain medication on warfarin metabolism and the INR.
